Thank you for considering Millennium Sound for your upcoming event.  If you decide to hire our services, please complete this form and return it in the enclosed envelope AS SOON AS POSSIBLE, so that we can hold your date!  After we receive this completed form, we will prepare the contract (2 copies), and return them to you for your signature.  You should then keep one (1) copy of that contract and return the other copy to us with your $100 down payment.

EXTRA SERVICES

Slide Show Power Point ($35) - We can provide equipment for a picture slide show during your reception with a large screen presentation and music.  We can set up your slide show for additional costs.

DJ Tent (Free) - For your outside event, we can provide a white tent to cover us for your outside reception.
